10/29/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, client was content. QMHP engaged mother in a discussion about his behaviors presented since the last session. Mother verbalized to QMHP on how she struggles with him implementing positive coping skills to assist with managing his anger 2-3 times daily last for 2 minutes until he is calm. Behavior: During second portion of the session, client needed assistance with able to manage his ADHD as he got up several times within the session playing with his sibling and getting upset with his mother when redirected back to the session as he was screaming to the top of his lungs until being redirected 1-2 times during the session lasting 2-3 minutes per second session. Intervention: QMHP inquired about weekly activities, asking what he had planned and what he looked forward to that would bring him closer to his goals. QMHP inquired about habits and routine that encouraged client to be productive and attentive in his daily life functioning. QMHP encouraged client to make good choices and to utilize self-calming techniques (i.e. counting to 10 and deep breathing skills) and processed with him about his behaviors, the good and bad, as well as, individual expectations. QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to emphasize with others. Intervention: QMHP reflected on his weekly plans and what new things transpired from client being a part of the ADHD population. QMHP held consultations with client and mother about effective intervention strategies that build on the client's strengths and attributes. QMHP explored the clientâs capability to cope with his stressors at home without depending on his therapy. QMHP provided praise when the client continued to display positive behaviors throughout the day, remaining on task and was very respectful to siblings and mom. QMHP encouraged client to continue having a good day. Response: Client was seen having a good time. Client displayed respectful, non-aggressive behaviors while discussing key factors related to his disorder. Client avoided distractions that were happening around him, due to utilizing social management skills. Client communicated the do's and don'ts of home rules so that he shows more commitment to his own decisions. Client was able to identify the role he plays in each situation and continues to comply with authority rules and regulations. Client was respectful of others and careful not to cross boundaries and personal space. Client was in a pleasant mood going into the second half of his therapy. Response: Client implemented the positive reinforcement provided by his counselor and was able to stay motivated and calm throughout the session. Client took into perspective the selected interventions that were chosen. Client agreed to behave and communicate a positive attitude when executing problem solving and self-control strategies. Client utilized communication cues in efforts to display the expected behaviors needed from him. Client promised he would try his best to correct himself when faced with conflict or a personal issue. As the client has made more of his own decisions to help maintain a healthy routine, he reported increased satisfaction and function.
